COMMAND ENDORSEMENT COMPLETION INSTRUCTIONS.
Command Endorsement is required for all Active Duty Service Members and Reservists in an Active Status. To be accepted, the form must be DIGITALLY SIGNED WITH CAC CREDENTIALS by your supervisor; an E-7 or above (CMC/SEA/DLCPO for enlisted; or CO/XO/DH for officers) or equivalent for other Military Branches. Incomplete, hand written, or forms without valid digital signature via CAC credentials will not be accepted and will further delay selection to attend the retreat.
Steps to properly complete the form:
  • On a desktop computer, download and open the PDF using Adobe Acrobat Reader.
  • When the yellow notification bar appears at the top, click "Enable All Features."
  • When the yellow notification bar appears again, click "Options" on the right side, then select "Enable JavaScript for this document one time only."
  • When the yellow notification bar appears a third time, click "Enable All Features" again.
  • Enter in the applicable retreat details, your rate, rank and name, etc.
  • Save the file and email to your supervisor.
  • Supervisor must complete their required fields, "Approve" or "Disapprove" your attendance and digitally sign.
  • Once returned to you by your supervisor, upload the file to the registration application you complete within this website.
